Let’s have a chat

Tell us what is happening, what you need a hand with and what a better week could look like.

We would love to meet you

Come as you are

You do not need to know the right service name, have every detail organised or arrive with a perfectly polished plan.

Maybe the housework is getting away from you. Maybe leaving home has started to feel like a bigger deal than it used to. Or perhaps you are looking for someone kind, reliable and easy to have around.

Contacting us for someone else?

Contact Horizon Care and tell us what life looks like at the moment. We will listen, ask a few helpful questions and see whether we could be a good match.

Reach out for a chat

Reaching out to a new support provider can feel like a lot, especially if you have already spent weeks making calls, repeating your story or trying to work out who might actually understand.

We keep the first conversation simple.

You can tell us what has been difficult, what kind of personality you feel comfortable with and what you would like more of in your life. That could be more independence at home, more confidence in the community or simply a bit less stress in your day.

With eight years of hands-on support experience, we know trust does not appear after one phone call. It grows when someone listens, remembers the little things and follows through. That is the kind of beginning we aim to create.

Not sure what to ask when you contact Horizon Care? Start here. We have answered some of the questions participants, families and referrers commonly have before getting in touch.

We start by learning about the person, not just the support they need. We ask about their routines, personality, communication preferences, goals and what has or has not worked before. From there, we are honest about whether our experience, availability and approach are a good match.

Yes. Feeling comfortable with the person coming into your home or joining you in the community matters. An initial meeting gives you the chance to ask questions, get a feel for their personality and decide whether the connection feels right before anything regular is arranged.

Absolutely. With your consent, Horizon Care Support Service can work alongside the people already involved in your life. That may include family members, carers, support coordinators, therapists or other providers. Clear communication helps everyone stay on the same page without taking your choices out of your hands.

Yes. Some weeks may need more structure, while others may call for a lighter touch. We understand that energy, confidence, appointments and everyday life can change. Your support can be adjusted where possible so it still feels useful rather than rigid.

You do not have to retell every detail, but it can help to share what made you feel uncomfortable, unheard or let down. When you contact Horizon Care, we will listen without judgement and use that information to understand your boundaries, preferences and what would help rebuild trust.
